J-1 Waiver CONRAD State 30 Program

Alabama – Program starts October 1, 2012 and continues until all slots are filled. As of February 19th, 2013, 10 slots remain for primary care. No slots remain for sub-specialists. 

Alaska – Program starts October 1, 2012. No deadline for slots. As of April 8th, 2013 20 Primary Care and 10 flex slots are available.

Arizona – Program starts October 1, 2012.  Note: Primary Care and Psychiatrists must fill 22 of the 30 slots. Applications are accepted until slots are filled.  

Arkansas – Program starts October 1, 2012. Applications are accepted on a first come, first serve basis until all 30 slots are filled. Arkansas accepts primary care and specialists applications. As of April 8, 2013, 11 slots are available, 1 of which are non – designated slots.

California – Program starts October 1, 2012.  CA accepts applications only for primary care physicians until June 30th. If the slots are not filled by the deadline, specialist physician applications will then be considered. California usually fills all 30 slots with primary care physicians before June 30th. As of March 7th, 2013 11 slots remain.

Colorado – Program starts October 1, 2012.  Ten of 30 slots can be FLEX. Colorado has 10 specialty spots available per year. This program accepts applications on a first come, first served basis until all slots are filled.

Connecticut – Program starts October 1, 2012. Connecticut accepts specialty physicians and there is no priority for primary care physicians. This program accepts applications on a first come, first served basis. They will continue to accept applications until all 30 slots are filled.

Delaware – Program starts October 1, 2012. Applications are accepted until June 30th, 2013. Specialist physician applications will be considered if the facility can document the need for the specialty. As of April 12th, 2013, 18 slots are available. Slots are filled on a first come, first serve basis.

District of Columbia – Program starts October 1, 2012. Ten slots are exclusively for primary care physicians and twenty slots are used for both primary care and select specialty physicians. Deadline is until all applications slots are filled.

Florida Program starts October 1, 2012Applications are accepted until November 30, 2012. Florida is closed as of February 14, 2013.

Georgia – Program starts October 1, 2012.  Applications are accepted until all 30 slots are filled or the start of a new fiscal year. Speciality slots that are not filled by the deadline of Apr 1, are available to primary care or specialist physician on a first come first serve basis.

Hawaii – Program starts October 1, 2012. Applications are accepted until all slots are filled.

Idaho – Program starts October 1, 2012. Applications are accepted on a first come, first serve basis until all slots are filled. As of April 12th, 2013, 29 slots are available for Primary Care Physicians.

IllinoisProgram starts October 1, 2012.  Illinois accepts specialty applications; the rules reserve 12 slots however this is dependent on the applications received. Applications will be accepted again from January 1-30 2013. As of April 11, 2013, No slots are available.

Indiana – Program starts October 1, 2012. As of January 2013, no slots remain.

Iowa -Program starts October 1, 2012.

Kansas – Program starts October 1, 2012. Primary care physicians are prioritized but specialists are considered. No deadline. If an application is received and cannot be processed by the end of the current cycle, it will be processed for the following cycle.

Kentucky –  Kentucky’s program accepts applications for each year from Oct 1st to Oct 31st. There is not a limit on specialty physician applications as they are reviewed according to the state’s need.

Louisiana – Program starts October 1, 2012. Each year applications for sub-specialists have 6 slots reserved from Oct 1st to March 31st. Applications are accepted on a first come, first serve basis starting April 1st. However, specialists are required to complete the Dire Need Criteria form with their packets which is then evaluated on an individual basis. As of March 7th, 2013; 26 slots remain.

Maine – Program starts October 1, 2012. Five slots are FLEX. Unlimited specialty physician applications are accepted, and the deadline to file is until all slots have been filed. As of April 12, 2013, 18 slots are available.

Maryland –  Program starts October 1, 2012.

Massachusetts– Program starts October 1, 2012. Specialty applications will be accepted at the beginning of January 2013. As of April 16th, 2013, applications are accepted till all 30 slots are filled.

Michigan – Program starts October 1, 2012.Michigan does not have a limit on specialty physician applications, but primary care applications receive priority.  All slots have been filled for  2013. Michigan’s Conrad 30 application packet for 2013-14 is available. The 2013 application submission window is October 1 – November 27th.

Minnesota –  Minnesota is accepting applications until all 30 slots are filled or until the deadline of Sept 30, 2013. Specialty applications are accepted and there is no limit, however, the state reserves the right to change this depending on primary care demand.

Mississippi – Program starts October 1, 2012. Deadline is until all 30 slots are filled. As of April 15th, 2013 25 slots are available.

Missouri – Program starts October 1, 2012. As of April 15th, 2013, 1 slot is available.

Montana – Program starts October 1, 2012.  Deadline is until all slots are filled. As of April 19th, 2013, 28 slots are available, and 3 specialty slots.

Nebraska – Program starts October 1, 2012 . Deadline is until all slots are filled. As of April 18th, 2013 10 non shortage area slots are available. There is no limitation on slots filled by specialty type.

Nevada – Program starts October 1, 2012. Specialist applications are reviewed on a case by case basis, no defined limit.

New Hampshire – Program starts October 1, 2012.  As of April 19th, 2013 18 slots are still available, of which only 4 flex spots are remaining.

New Jersey – Program starts October 1, 2012. New Jersey accepts applications until all 30 slots are filled or the deadline of September 30 of each year. Slots roll over into the next cycle. Specialty physician applications are accepted for State Designated Trauma Centers and Children Hospitals. As of April 19th, 2013, 30 slots are available.

New Mexico – starts October 1, 2012The New Mexico J-1 Waiver Program is a first come first serve program. As of September 27th, 2013, 21 slots are available; 1 non-designated area slot is available. Applications received after all 30 slots have been filled will be returned. J-1 Applications for 2014 will start being accepted in Mid- September. 10 non – designated area waiver applicants are allowed annually. non -designated area waivers are not being accepted at this time. However, designated area waiver applications are being accepted.

New York – Deadline to submit applications was November 30, 2012. As of April 19, 2013 all 30 slots have been filled.

North Carolina – Program starts October 1, 2012. North Carolina reserves 10 spots for specialty physicians. As of April 19th, 2013, 17 non specialty slots are available and 5 speciality spots.

North Dakota – Program starts October 1, 2012. Applications are accepted year round. There are 10 slots are always filled for physicians that work outside of designated shortage areas. Usually these are specialty physicians although primary care physicians have been placed in these areas. As of April 19th, 2013 14 slots are available. No slots are available for specialty at this time.

Ohio – Applications for primary care physicians located in health professional shortage areas will be accepted Oct 1st to June 30th. Applications for primary care physicians not located in HPSAs and all non-primary care physician applications will be accepted Jan 1st through April 30th. Emergency community need applications will be accepted July 1st through Aug 31st. Ohio accepts and does not have a limit on sub-specialist applications.

Oklahoma – Program starts October 1, 2012. Primary Care is given priority but specialty applications are considered at a case by case basis and 10 applications are accepted for specialty physicians. Deadline to file is until all slots are filled or Sept 30 of every year.

Oregon – Program starts October 1, 2012. Specialty applications are accepted with 6 slots reserved for Specialty physicians, however that number may change based on the need of the state for the fiscal year. Deadline is until all 30 slots are filled. As of April 25th,2013 6 slots are available. However, overflow applications will be submitted on October 1st.

Pennsylvania – Program starts October 1, 2012. As of April 23, 2013, 8 slots are remaining.

Rhode Island – Program starts October 1, 2012. Specialty applications are accepted, however the emphasis is placed on Primary care physicians who speak spanish. Applications are accepted till all slots are filled.  As of April 23, 2013, an undisclosed amount of slots are still available.

South Carolina -Program starts October 1, 2012. Unlimited specialty physician applications are accepted. South Carolina accepts applications until all 30 slots are filled.

South Dakota – Program starts October 1, 2012. South Dakota accepts applications throughout the year and no limits are placed on specialists applications. As of April 23, 2013 19 slots are available. The open slots can be filled with primary care physicians or specialists.

Tennessee – Program starts October 1, 2012. Applications are accepted until all 30 slots are filled . 

Texas –  Applications for the upcoming cycle 2012-2013 will be accepted beginning September 5, 2012. Texas accepts applications until all spots are filled and on a first come, first serve basis.  Texas is closed as of February 14, 2013.

Utah – Program starts October 1, 2012. Up to 10 slots are accepted for sub-specialists. Applications are accepted on a first come, first served basis, until all slots are filled. As of April 23, 2013, 30 slots are available. Specialty is based on the employment site documenting the need.

Vermont – Program starts October 1, 2012. As of April 25, 2013 there are 8 flex slots available and 18 regular slots.

Virginia – Program starts October 1, 2012. Applications are accepted year-round on a rolling basis. Primary care physician and specialty physician applications are accepted but preference is given to primary care. As of April 25, 2013 there are zero slots available.

Washington – Program starts October 1, 2012.  Applications are accepted until all slots are filled.

West Virginia – Program starts October 1, 2012.  Zero slots are available at this time.

Wisconsin – Program starts October 1, 2012. Applications are accepted on a first come, first serve basis until all 30 slots are filled and there is no cap on the number of specialists they accept. As of April 25th, 2013 14 slots are available. No distinction is made between HPSA v. non-HPSA areas. 7  slots are available for non-HPSA areas.

Wyoming -Program starts October 1, 2012. Five slots can be used for specialist positions. Applications are accepted until all 30 slots are filled.
