The United States Citizenship and Immigration Services (USCIS) has made changes to various forms and revised versions are now available for use. We would like to remind you that previous versions of the forms listed below will no longer be accepted after the dates noted below.
We recommend physicians performing medical examinations to start using the new Form I-693 without delay. Applicants, undergoing medical examinations should verify that their physician is using the most recent version of Form I-693.
